How do I stream with Adobe Flash Media Live Encoder?

I'm new to Twitch.tv, and they changed the site up a few months ago, so I don't know how to stream here anymore. I used to be able to with my friend using Manycam on his account, but now I can't. So how do I use FME for streaming a Dazzle DVC 100 feed onto Twitch.tv?
